Please wear clothing you can move comfortably in during your appointment. 

If needed, we provide shirts and shorts for you to borrow. 

Locker rooms are available for changing and storing your belongings. 

A charging station is available if your phone is running low.

Please keep your phone on silent to ensure a good appointment for yourself and others around you.

Prior to your first visit, we will send you the forms needed to be filled out prior to your visit via email. To help expedite your first visit, please complete the forms sent by email.  If you choose to fill out the patient form packet at our office, please arrive 15-20 minutes before your appointment to allow enough time to get them completed before your appointment.

The main building's doors are locked before 8:00 AM and after 5:00 PM each day, as well as on federal holidays. If the doors are locked, please use the call box by entering the code "200", then pressingthe "Call" button to be buzzed in.
